Employee Authorization Form Requirement
To protect consumer privacy and ensure compliance with federal regulations, Truework requires a valid Employee Authorization Form for all employment and income verification requests. This signed form confirms that the individual has granted explicit permission for their information to be shared with a third-party verifier.
The authorization form must be:

Signed and dated by the employee within 90 days of the verification request

Obtained and submitted by the requester (the person or organization initiating the verification)

Uploaded prior to the start of the verification process
We accept both electronic and physical (wet) signatures. If the employer specifically requires a wet signature, we will contact you directly to request an updated version. Truework does not mandate a specific format for the authorization form; if you do not have one available, you may download a suitable template online.
