An employee authorization form is a signed form that clearly states the employee has authorized you to perform an employment verification on their behalf. It must be obtained by the person submitting the verification request and is required for all requests. We will not start working on the verification until after we have received it.
This form must be signed and dated by the individual within 90 days of the request being submitted. We do accept electronic signatures, but if the employer requires a wet signature, we will reach out to you directly for a new one. We do not require a specific format for the form, so if you do not have an authorization form on hand, you can download a template online.
If the form was not uploaded when you submitted your request, we will send you an email with instructions on how to add it to our system.
